To enroll your child you will need the following information.

Verification of age:
(one of the following)
Birth Certificate
Insurance policy
Passport
School record
Certificate of baptism, accompanied by parent's affidavit
Bona fide Bible record, accompanied by parent's affidavit
Affidavit of age sworn to by parent, accompanied by a medical practitioner's statement

Extended Day

Azalea Park offers an Extended Day Enrichment Program both before and after school. The Extended Day Enrichment Program offers supervision and a variety of enrichment activities for elementary-age children. The program is fee-supported by those using its services. Daily activities may include homework time, supervised free play, art, movies, and organized games. Your child must be registered for the program to participate. 

Hours and fees:
From school dismissal until 6:00 pm is $30.00 a week.
From 6:45 am until schools starts is $15.00 a week.
Package deal: AM and PM is $40.00 a week.

School Advisory Council

The School Advisory Council (SAC) works together to implement our School Improvement Plan.  As an important part of our school, this Council helps to build the framework needed in order for our school to be successful. The council is made up of both staff and parents.

Supplemental Education Services (SES) Tutoring

Provider Fair

Date:Wednesday, August 20
Place: Multipurpose Room
Time: 4:00 – 7:00 pm

Your child may be eligible for free tutoring. If your child receives free or reduced lunch, you can come this afternoon and meet with the different providers and choose a company for your child.

This program is available to grades K through 5th. The tutoring will be held on Monday and Friday after school from 3:30 to 5:30 pm. Parents are responsible for providing transportation after school. If transportation is a problem then you might want to consider choosing a company that does tutoring in your home.

This is a great opportunity for your child to get the extra help they might need!
